Upstate's Intensive Transitions Team will discuss their work.  This team is responsible for working with those patients who are most likely to be readmitted in the 30 days after discharge.  You will learn how they identify these patients, what happens when these patients leave the hospital, and what you can do as a healthcare provider to help make an impact.

If you know of anyone else interested in a collaborative approach to healthcare, please be sure to alert them to this "can't miss" discussion.

For additional information, please contact Darren Carboni, Director of Interprofessional Education.
